How to Register

- Registration

Registration is limited to the first 550 to submit completed forms and fees.
General Registration Information

* Everyone attending the Retreat, including chaperones and volunteers, must complete the Liability Release Form, Code of Conduct and Travel Form.

*   All forms for participants age 17 and younger must be signed by a parent or legal guardian and must be completed in advance of the weekend.

*   All are encouraged to pre-register.

*   Participants, age 17 and younger, who wait to register at the door must come with their chaperones, and st have their Forms signed by a parent or legal guardian and will only be allowed entrance if space is available.

* Return completed registration forms by OCTOBER 17, 2019.

* FEES: $60 per youth ($30 per adult). The Group Leader is responsible for collecting fees from each member of their group, and submitting only ONE CHECK per group, payable to ABLAZE 2019. Please write group's name on the check.

* SAFE ENVIROMENT: All chaperones must submit proof of CURRENT compliance with Diocesan requirements for providing a safe and secure environment for minors (see SAFE ENVIRONMENT on this web site).

*Deadline: Mail by OCTOBER 17, 2019.
* MEALS: Fees include lunch, dinner on Saturday, snacks and a drink with each meal.
How to Register

* If you are attending as part of a Group (parish/family/school), submit all completed forms and fees TO YOUR GROUP LEADER.

* GROUP LEADER:
1. Email ablaze.retreat@gmail.com to request a Group List Registration form. A link to a Google Document will be sent to you to complete.
2. Collect ALL permission forms for your entire group (youth and adults) and mail the completed forms in one envelope, together with ONE CHECK for your entire group, payable to ABLAZE 2019. Please write group's name on the check. Mail by OCTOBER 17, 2019, to:

* The Group leader is responsible for collecting all the fees for their group and submitting only ONE CHECK for the entire group to ABLAZE 2019.
